Contact your insurance agency as quickly as possible and report the accident. Regardless of fault, your insurer is responsible for providing access to health and rehabilitation services. If you were a passenger in someone else’s car or involved as a pedestrian, contact your own auto insurance provider (if applicable). Your adjuster will determine which insurance policy will handle your claim. They will send you a package of documents, which you will need to complete to start your claim for accident benefits. You must complete and return the application for accident benefits (OCF-1) to the insurance company. It is mandatory that you submit the OCF 1 to your adjuster in order to get approved for any treatments you require.
2) Initiate a Claim and Book an Initial Assessment
Once your injury claim is initiated, book a one-hour physiotherapy initial assessment with us.
